Penile Extenders 101

This article is designed to introduce penile extenders and explain some major points that are rarely if ever talked about regarding penile extenders by the companies that sell them. There are many things that are left up to the buyer to figure out, and with almost 4 years of extending behind my belt (literally), I have written this guide section to address basic points on functions of penile extending and how it works.

About Penile Extenders

First of all, let it be known: Penis extenders do work! They really can make penis enlargement a reality. However, they have been starkly misrepresented by the companies that sell them in many ways.

penile extenders all work on the same principle by providing spring loaded traction to the penis shaft over time to induce mitosis in the cell tissue of the penis, thus lengthening the shaft of the penis.This mitosis (or hyperplasia) is when cell tissues actually duplicate themselves, divide, then multiply due to being put under tension over a long period of time.

This is something the penile extender companies do actually tell you, then they all show you pictures of indigenous peoples who have stretched their front lip or ear lobe to hold some ridiculously sized wooden plate like object, or they show the women who have stretched their necks a full foot. Good sales pitch? I would rather see something more on topic, like perhaps actual info on what the product can actually do to enlarge the penis.

Before We Begin, Penile Extender Types Vary

Now, it’s important here to point out that there are mostly 2 different types of types of extenders. There are extenders that have different cradle-harnessing devices, those being the “noose” design, and the more well known “strap” design.

Another difference in extenders is whether they have the ability to put the cradle on your penis and then attach it to the extender, or not. The other, more common types of extenders that do not offer this feature make it necessary to put the extender on with the cradle or harnessing device already attached to the of the extender. This means that the extender's cradle is already attached to the extending rods and to the base of the extender.

So the most common 4 types of extenders can be separated into these possible categories.
1) Noose harnessing cradle with “removable” cradle
2) Noose harnessing cradle without “removable” cradle
3) Strap harnessing cradle with “removable” cradle
4) Strap harnessing cradle without “removable” cradle

There are also some other types of penile extenders like vacuum methods for fastening the glans to the extender, among others, but for this article, I have chosen to address the #4 type of extender. This is the type of extender I have used. Also, of the interviews I have done with many here from the PEGym forums, having to do it “all at once” without a removable cradle seems to make the #4 type extender the most challenging type of penile extender to learn to use properly for optimal gains.

Problem: What Penile Extender Companies Don’t Tell You.

One problem is that many men have spent money based on incomplete truths. This is especially true with regard to the total extender wearing “Time/Gains ratio”.

A major example is the little “calculator” that many of the extender websites have up on the site that allows a potential customer to enter their “current size” and “amount of time they plan to wear the extender” and then it will give you a readout telling you how much you can expect to gain.

I typed in 7 inches on several of the sites and most of them told me I would be at 10 inches by the end of 6 months. That would be amazing, but this is simply not true and is a ludicrous tactic used to sell the product.

There are some main points/dilemmas with extenders that guys are discussing on this forum such as:

  • Looking to better understand physical factors like numbness, coldness, and what they mean.
  • Looking for help with a routine and more how to wear the extender.
  • What to expect in terms of time spent wearing an extender and ex

    pected/realistic gains.

The Truth about the Time/Gains Ratio.

The simple truth is, it takes a long time to see the gains you likely have in mind. Face it. With my personal data I have conjectured that it takes at least 1,000 to 1,200 hours of proper extender use to see a half an inch gain. Let’s think about this. Say you could wear the extender for 24/7. This would take between only 6-7 weeks of wearing the extender device to gain a ¼  to ½ of an inch.

Over the 4 years of using my penile extender I have found that the best amount of time per day to wear the extender is between 4-5 hours a day. So to gain one inch it will take about 500 days of wearing the extender for 5 hours per day, or better put, well over a year and a half of wearing your extender every day.

The major problem exists that there is no way to possibly wear an extender 24/7. You will find it exceptionally difficult to properly wear your penile extender for over 2 hours without your glans getting cold, numb and losing circulation or, your shaft skin to begin to have a burning sensation.

You will have to devote serious time and serious effort in a more or less private situation over a span of years if you plan to gain permanent inches of length to your penis.

Concealment and Wearing a Penile Extender in Public

Extenders cannot be worn under your pants safely in public or with the proper amount of tension for gains. I myself and many of my friends with extenders agree. It never works, and could possibly either cause injury, embarrassment, or both.

Why?

You have to be able to see and adjust the extender with its screw-rods incrementally in intervals of around 15minutes or so each in order to maximize the best stretches during one session alone and to be sure you are not going numb and losing circulation.

This is why the side-rods are able to be adjusted, so that once your penis gets accustomed to the stretch you initially get when you simply “put it on” for a session, it will not be enough during the stretching session to maximize gains, or in my opinion, make any real gains at all.

Penile Extender Safety Concerns

If the extender gets too tight, you must conversely let some tension off of the extender.
This means it may take a full hour just to get to a “set” tension. This in turn means that you will have to do this where you can be pretty private, or where no one cares what you are doing.

Extenders can really only be worn under clothing such as a very loose pair of sweatpants or something that will provide plenty of space. The mere space provided by loose jeans or slacks is not enough for proper penile extender use.

In addition, the main reason why you would never want to do this is due to safety concerns. Just as the extender needs to be adjusted for max gain potential, it also needs to be able to be adjusted at a moment’s notice in case your glans goes numb, you start to feel pain, or the extender just begins to come off, among other miscellaneous reasons. It is normal for the glans to become slightly cold, but if severe cold or numbness is experienced, the extender should be taken off by the user immediately!

Because of these common situations, public extending is highly discouraged. Every extender on the market is just too bulky as of now, and I still do not find public extending to be a good idea if one is serious about making gains.

How to Use a Penis Extender

The problem with regarding “How to Use a Penis Extender” instruction guides up to this point is that many such guides do not give thorough information on what an extender user should do and experience to know they are on the right track towards optimal gains. This guide article covers the major points on how to do so. While there are some basic principles of “acquiring and mastering useful techniques” with your extender, it took me years to figure it out on my own. This article clarifies much of the guesswork of how to use penis extenders.

Applying the Penis Extender: “Strapping-In”

One of the first things to consider when you are about to begin using your penis extender, is whether or not you have a pronounced frenulum. Whether you are still intact, or if you are circumcised, maybe you have a substantial frenulum.

The frenulum can be very sensitive on some men, in which case a piece of cloth or gauze is worn to protect the frenulum from the hard surface of the extender’s cradle.

Gauze that sticks to itself is usually preferable because it stays in place once applied. A strip about 6-8 inches works well to make 2 complete revolutions around the penis just below the glans.

Now, this 4-step process is how the penis extender is applied to the penis.

  1. Make sure one side of the strap is fastened to the cradle of the extender, so that the strap will stay in place when applying the extender onto the penis. Usually the preferred method is to leave the strap unattached on the side of the extender that corresponds to your dominant hand, so if right handed, leave the right side of the strap unattached, and vice versa.
  2. Then, grab the penis just below the glans and with your opposite (non-dominant) hand and pull the glans towards the cradle of the extender. Once in place and with a grip still holding just under your glans, slide the strap up and over the penis behind the glans where it feels comfortable. Some men prefer some extra “slack” skin to be above the grip point of the strap for added comfort and to better protect the glans and frenulum.
  3. Now put the strap through the opening for it on its side of the cradle and secure it tightly while still keeping the penis held in place still by the other hand. Next, secure the other side of the strap to the extender with the dominant hand, making sure to keep enough tension on the penis so it stays in place and the glans does not slip out of the grip from the strap.
  4. Finally re-tighten the strap 2-3 more times on each side (one side at a time) to ensure a tight fit so that the glans in no capacity can slip out from the grip of the strap. Also be careful to not pull the strap so tight that it pulls shaft skin though the opening for the strap. This will be very painful, and you should feel it at once if this begins to happen.

Wearing the Penis Extender: The Waiting Game

Once everything is fastened and secured and you are well “strapped in” to the extender, as many say, it’s mostly an art of maintaining a comfortable stretch and doing things you enjoy to do while sitting or lying down. Much of the writing this article was done while wearing my extender. Many just enjoy watching TV, using their computer/laptop, or doing just about anything that does not require physical movement.

Some attest that lying almost flat on your back is the best way to keep the glans swollen with blood hence providing the best form of resistance for stretching against. This has come to b

e known as a “Head-Swell”, and by most is considered to be the one key factor in optimizing gains with extender use. How to get and maintain a “Head Swell” is fully covered in the following section of this guide.

The key here is to not move around so much that you loosen the grip of the harnessing strap, thus weakening the tension applied to stretching the penis. You obviously never want to over stretch, and sheer pain is the obvious best indicator of too much tension. You should be trying to get the maximum amount of tension you can without pain from the extender when wearing it. This is what causes gains to become maximized.

Tension + Time = Permanent Penis Shaft Length Gains.

Maximizing painless tension while wearing the extender is what helps optimize your gains.

This is done by first getting the extender on well so that you get a “Head Swell” and then by using the screw tension rods that connect the base of the extender to the glans cradle. Once the extender is secured well and the Head Swell is achieved, you should begin to slowly screw out the rods applying more tension to the shaft of the penis.

Personal research and interviews yields that the best way to do this is to twist the rods 3 revolutions every 10-15 minutes. You do not want to tighten it all the way out to quickly, because the penis shaft needs time to gain more plasticity and trying to do it too fast will cause pain and a loss of head-swell. Applying heat is always a good idea and very helpful to the process of penis extending if possible.

Applying Heat while Wearing the Penis Extender

You can apply a warm rice sock to the penis shaft while extending in order to not only help the penis achieve a more plastic/malleable state more quickly, it also helps reduce the time for extending the extender rods. This allows you to achieve maximum tension, or your BPXL (Bone Pressed eXtender Length) more quickly.

Applying heat also helps when you begin using an extender because it can help prevent injury. The same heat sources recommended for manually exercising the penis can be used while using an extender as well. However, liquid heat should never be used, as it could contribute to oxidation (rusting) of the metal parts of the extender. It can also help add to chaffing if you are using gauze or cloth as a barrier between your penis and the extender.

Optimal Maximized Tension

Once you have screwed out the side rods so that no more tension can be applied to your penis without any pain, stop. You are now at the point where you have max tension and gains are optimized. Trying to overdo it from this point will only result in injury, hence detracting from the purpose of wearing the extender.

In the beginning, never over extend the tension rods. Doing so can cause pain and lead to injury. It can also destroy your extender if you are not careful. As extenders are not cheap products like some other penis enlargement devices, it is important to be careful with your extender and not explore the limits of its resilience. They are fragile and thus is why public use is not recommended.

Take care of your extender and it will continue to give you the gains you desire. Although it takes time, penis extenders are proven to work and will definitely deliver permanent enlargement of the penis!

The Head Swell: Maximize Your Penis Extender Gains

This section covers a particular phenomenon that can occur while wearing a penis extender called a “head-swell”. It is now believed by many that the head-swell helps to optimize penis shaft length gains while wearing a penis extender. However, some disagree or say that a head-well is not necessary for making gains when using a penis extender. Nonetheless, this article focuses on how to get and maintain a head-well while wearing a penis extender.

What is a Head-Swell?

Best defined, a “Head-Swell” is a blood-engorged glans that occurs while wearing a penis extender which allows the penis extender to be worn at appropriate tension levels. Appropriate tension is best defined as enough tension so that there is level of tightness in the middle of the penile shaft when the extender is properly worn with a head-swell.

A head-swell is also believed to help the penis from going cold or numb while wearing a penis extender, since it means that the glans will be full of blood. This is not proven, but I have noticed that indeed, a head-swell does seem to allow me to wear my extender for a longer amount of time without cold or numbness occurring.

I am a firm believer that using a penis extender is different for every man, and that it is not an exact science (yet). Each man must figure out what works best for him, but the ability to get a head-swell is a very good skill to possess if one is planning to use a penis extender.

How to Get a Head-Swell: Proper Application is Key

Let us review quickly how the extender is applied, which will segue into obtaining a head-swell. Remember, proper application is the first step to achieving a head-swell! You will begin to notice that the better you become at putting the extender on with a good deal of blood already in your glans, the easier and better your head-swells will be.

First, get about a 30% erection, and have your extender ready to apply. Now, do some helicopter shakes and make sure your penis is not at all “hard” per se, but is not turtled in any way either, and is moderately engorged with blood. Good, long and loose is what you want.

As a reference point, my penis will only stay like this for literally a matter of seconds before it loses all “flushed flaccid” characteristics which is why having you extender and any wrapping (gauze or cloth) ready to apply.

Head-Swell Focus

Put the extender on by grabbing mid shaft to a quarter length of the shaft from the glans and pull some loose skin of towards your glans in a somewhat jelqing fashion, keeping as much blood in your glans possible. At this point secure your fastening strap to the shaft.  Then secure the penis to the extender and cinch down this right side.

Now, having the right side somewhat pretty tightly fastened, unfasten the left side and re-fasten and re-tighten the left side of the extender. At this point, the left side is only barely secured to the extender to give you something to pull against when setting the initial right side.

Next, make a final adjustment to each side and when you can no longer pull the strap, you are “Strapped-In”. You ar

e now ready to focus on the final steps of achieving a head-swell.

Kegeling and Jelqing to Get Head-Swells

Immediately after you get it on, one method that is most common is to use the kegel to force even more blood into the glans. Do this by simply kegeling once you have the extender on, but also use gravity to your advantage by making sure that your penis and the penis extender are pointing downward while you begin kegeling.

This generally works well, but I have found that there is a portion of times that kegeling will not effectively produce a head swell. This is when I thought, “Why not try to manually use a jelqing motion to get more blood in the glans to better create a head-swell?”

So, I have found that even if I do not get the extender on with adequate blood in my glans at all, and if kegeling fails to force blood into my glans, by using my thumb and forefinger to do a quasi-jelq up the shaft with the extender on I can get a head-swell. You should also be very careful as to not touch the side bars while jelqing a head-swell or you could break your extender by squeezing the rods. Extenders are expensive! So take special care to only touch your penis shaft when attempting the “Jelqing a Head-Swell” maneuver.

Keeping a Head-Swell and Proper Tension

As it may be best for me to fasten it standing up, maintaining a head-swell is hard while standing for long periods of time. Some believe that lying down is best for keeping a head-swell. I have no problems maintaining a head well while sitting up, but standing for too long or walking around can surely cause a good head swell to diminish. Try not to move around too much once you have the extender securely on your penis with a good head-swell.

I do well also by checking frequently to see if I am losing the head-swell. If I am, I simply twist/screw-in my torsion screw bars so that they are back in their original (shortest) position, then without taking the extender off, I simply re-jelq my head-swell, and it generally works well. If not, you will just have to take off the penis extender and wait a while before trying to reapply it.
Also remember that it takes practice.

Proper use of a penis extender can be much like learning to play the guitar, or use a tool well. It takes some time to get accustomed to wearing your penis extender flawlessly every time. And even then, just when you think you have it figured out, we all have a problematic session every now and again. Just don’t get discouraged and stick with it!

Developing a Penis Extender Workout Routine

This section of the penis extender guide explains the necessity of creating a penis extender routine that will fit into your schedule. This section also will cover how you should track your extender sessions and gains with a log, as well as the best ways to incorporate using an extender with your manual penis exercises.

Now What?

Now that I have covered how to properly use a penis extender, it is necessary to develop a routine based on your daily schedule, goals for gains, and proper implementation. Since this site is devoted mainly to the manual stretching and jelqing techniques and exercises, I find it safe to assume a vast majority of the readers here are mainly looking for a way to add extending to your PE routine already in place, so this will also be covered. First, there are some things to consider.

Finding Time

If you will recall from the first section, extending can be quite arduous and in my humble opinion and for safety reasons, using a penis extender should really not be done while driving or in any public situation because of possible injury as well as cheapening your gains and wasting your time with not enough tension. Let me put it another way: If you can walk around normally with your extender on, then you are most likely doing it incorrectly or in a fashion that will not produce maximum gains for the time spent extending.

This means that you will have to find some quasi-private time when you can wear only a towel, very loose sweat pants or preferably nothing at all while you use your penis extender. Now, the fact that you have the time to be here reading this article/website generally assumes that you have at least a few private hours to yourself by which you could possibly also be wearing an extender and making blog posts, as in, right now!

You Really Do Have the Time

Face it, you have the time to devote to extending if you can sit in front of a computer and read this article now in private, which is likely the case. Now, if you have already spent the money on an extender (they are not cheap) and you are reading this article, it shows that you must want to put in some time and get the most out of your penis extending sessions.

One great thing about extending is that you can occupy yourself with other things while you wear the penis extender, so multitasking is definitely a possibility. But don’t plan on getting in your daily run or weight room session while you use your penis extender, you just can’t do so. However, any activity where you will be sitting or lying down for 98% of the time is perfect time for using your penis extender.

Extender Routine 101

So we have already covered the main issues, those being that you will need at least enough privacy to open your pants every 10-15 minutes to check for numbness and increase/adjust the tension.
Now, you will want to consider what times are best and when you will be performing actions like sitting at the computer, watching TV or doing nothing at all, that is, times each day when you can count on being able to use/wear your extender.

The key element to keep in mind regarding these time periods for extending is consistency. Ther

e may be instances for “extra time” that you can get an extra hour here and there, but the key thing to remember with your baseline routine are the times that you know you will be able to use it.
And it’s ultimately up to you to decide if you want a bigger, thicker penis. With proper technique and patience, you will begin to notice your BPXL begin to increase, and that you can hold a head-swell at longer lengths for longer periods of time.

Combining Penis Extending with Manual Penis Enlargement Exercise Sessions

One thing many guys ask on the PEGym forums is “How can I use my penis extender in combination with my manual penis enlargement exercise sessions that I currently do?” Well, this is not an easy task, especially once you consider first all the time it takes out of your day to do the other necessary things like work, school, and housework. However, there are some even more important things to consider.

One thing that is important to consider is when best to use the extender in relation to manual exercises. I have found that it has made an extreme difference in flaccid hang and I imagine eventual gains by wearing my extender immediately after my complete stretching/jelqing sessions. This is because of how it keeps the penis in an extended state after a rigorous workout.

I also think that it is not bad to use the extender before, but to avoid injury I would not wear the extender an hour or less before a manual penis exercises, as to not put too much stress on the penis while exercising with adequate intensity.

The Power-Hour

One concept that bgstick18 and I have developed is the idea of the “Power Hour”. This concept is that of wearing a penis extender for an hour at full tension then taking it off. This is only advised for men that have used a penis extender for 6 months to a year and are well accustomed to extreme stretches. We believe that your BPXL should be at least an inch longer than your BPEL before attempting a power hour.

When doing a power-hour, the idea is to get your penis to its maximum BPXL immediately after putting on the extender and getting a head-swell. Sure, it may be possible to add even more tension throughout the power-hour, but the idea is that you get pretty close to max tension right from the beginning.

I have found that by doing this right after manual exercises is making a huge difference in the way my penis feels, and I hypothesize that maximum optimal gains will ensue! More research needs to be done on the power-hour and how it helps/affects overall gains.

Final Note

Your routine for wearing your penis extender is a critical element in achieving gains and should be a well thought out plan. By organizing the free time in your day you can really optimize the time spent wearing your penis extender. And by then incorporating your penis extender routine with a manual exercise regimen, it is believed that even more gaining potential will manifest.

Bone-Pressed eXtender Length: Understanding Your BPXL

Wearing a penis extender can be a complicated process. More over, understanding gains made with a penis extender can also be baffling because the difference in your overall BPEL is usually less than the length that can be reached while wearing a penis extender. The solution? Track your gains consistently and accurately by measuring your Bone-Pressed eXtender Length, or BPXL.

Bone-Pressed eXtender Length Explained

A man’s “BPXL” is a new term coined by PEGym community member Dangler. You might have seen the term on our penis forums involving penis extenders. The BPXL is the term we now use to describe Bone-Pressed eXtender Length. There are a few elements that come into play here other than just a simple measurement taken while wearing the extender.

The first thing that must be lucid is that you measure bone-pressed just as you normally would with any other measurement. Hence, you do not measure from the base of the penis extender; you measure from how far you can press the ruler into the pelvic bone just outside of the penis extender’s base.

The next important factor is to know when to take the measurement. The measurement is taken when you have been wearing the extender for an hour or more without increasing the tension, meaning that you can “hold” that BPXL for at least an hour without numbness. Coldness and discomfort are not factors in measuring for BPXL, and can be tolerated but not in excess.

To measure BPXL, push the ruler or measuring device into your pelvic bone on the outside of the extender base just as you would to get your BPEL.

Measuring BPXL and Logging Hours

From now on, you should measure gains made with a penis extender while wearing the extender. However, remember that this is now your BPXL (Bone-Pressed eXtended Length) and should not be confused with Bone-Pressed Erect Length, your Bone-Pressed Flaccid Length, or other forms of penis measuring.

The reason for these separate measurements is because it can help you see exactly the length to which you can extend as well as for how long you held that stretch as you track your penis enhancement gains.

Again, your BPXL is measured by two factors:

  • First is your bone-pressed length while wearing the extender.
  • Second, it is the maximum extended length that you

    can maintain for at least a whole hour without going numb or causing any real pain.

Remember, if you extend to a length that you can’t even hold for 5-10 minutes, then you are just cheating yourself, and you will eventually become discouraged. Not only is this lying to yourself, but cheating in this manner also will not produce or reflect real quality penis growth, which takes time.

It is best to have a clock beside you when measuring your BPXL, or use the clock on your computer if you make a log using software. I use an MS Excel spreadsheet with which you can implement a formula that will automatically add up the time that I spent using my penis extender.

It is also a good idea to make notes regarding any thing that happened during each session, like if you experienced any numbness, if you were able to increase your BPXL, or if you were able to wear your extender for a longer period of time at your current BPXL.

The Key: Consistent BPXL Measurement Procedures

One of the most important things to keep in mind when taking a BPXL measurement is how you performed your last measurement. Exactly how long did you wear the penis extender? Did you feel any pain when you last measured?

You should really use your best intuition to try and mimic the exact conditions of your previous measurement when performing a new, accurate BPXL measurement.

It is very important to keep in mind that you are not trying to impress anyone. You are enlarging the penis, and attempting to make penis shaft length and girth gains. To observe and track these penis enlargement gains is why using the BPXL measurement is a very useful measurement.

Your ability to add more and more tension to your penis while wearing the extender will always mean that your BPEL has increased. Tunica Hanging and More on Penis Hanging Angles

This penis hanging guide section covers more in-depth discussions regarding the use of various angles when hanging. This is especially pertinent information when shifting the focus of your hanging from the suspensory ligaments to the tunica, which is considered the main limiting factor with penis enlargement gains.

SUSPENSORY LIGAMENT MAINTENANCE

If you did not naturally start with your penis exiting your pelvis at 6’ Oclock, you will need to do some maintenance work until you ‘cement your gains’. This means, you will need to practice some of your penis hanging at least a few sets per week at a lower angle: straight down (SD) or between the cheeks (BTC) to maintain your ligament stretch gains.

If your suspensory ligaments tighten, pulling your package upward to a higher angle (e.g. 6.5 O’clock or 7 O’clock), you will need to go back to hanging BTC or SD again (at least part time) until this is corrected.

Eventually your penis will stay at this downward angle and your suspensory ligament stretch gains will be ‘cemented’. No one can really tell you how long this maintenance will take since it depends on your physiology and how long it took you to reach 6’Oclock as well.

Once you’ve maximized your suspensory ligament stretch gains, you’re ready to start targeting the penis tunica.

TUNICA HANGING WORK

The tunica is probably the toughest single structure in our penis. Through your hanging career, the tunica will slowly toughen and become thicker. As you stretch the tunica and permanently elongate it, elasticity will slowly be lost and making permanent tunica hanging gains will slowly become more difficult. Bib (aka Bigger), an expert on hanging recommends the RSDT fulcrum for tunica hanging deformation process.

TUNICA HANGING: Straight Out, Straight Up, Over the Shoulder

If you’ve just started hanging and your penis exits your pelvis at 6 O’clock (straight down), you might not benefit from SD or BTC angles as much as others. However, you should still focus on tunica hanging via  BTC for at least a month to make sure your suspensory ligaments are fully stretched.

Once you have confirmed you have no more potential ligament stretch gains, you can move to OTS (over the shoulder) or SU (straight up). These are probably the two best angles for someone starting out with their penis naturally exiting the body at 6 O’clock. To hang OTS use a plastic milk jug to protect your shoulder (otherwise the rope or nylon strap from your penis hanger will eventually dig in and start to hurt).

If you’re using an OTS angle for tunica hanging, alternate shoulders with each set. If you’re doing an odd number of OTS sets, finish off with an hanging angle in the middle such as straight up (SU). Once you’ve had at least six months of penis hanging experience, and your penis exits your body at 6 O’clock (straight down) you will be ready for advanced Tunica work, including fulcrums such as the RSDT.

THE “RICE SOCK DUCT TAPE” FULC

RUM FOR TUNICA HANGING

The RSDT (Rice Sock Duct Tape) Fulcrum is an excellent way to create tunica creep, micro-tears and eventually permanent deformation. By the time you’ve maximized your suspensory ligament stretch gains, you may have one year or more of hanging experience under your belt. The tissues of your tunica will be toughened and no longer easy to deform. The RSDT is an excellent fulcrum for breaking through plateaus and creating fatigue all along the entire penile shaft.

Effectively the RSDT is a rounded surface (like a small mound) with channels on the sides that help your penis shaft lay straight. Bib, an expert on hanging wrote an article, complete with pictures on how to create an RSDT here: How to Build the RSDT

The RSDT is a very intense fulcrum that will create fatigue all along your shaft. At the start, you will only be able to hang a fraction of your normal weight using this device. However, you’ll reach excellent levels of fatigue quickly, and reports suggest penis length gains with this exercise are exceptional.

Are You at Risk for Peyronie’s Disease?

The primary symptom of Peyronie’s (pronounced pa-ro-NEEZ) disease is an abnormal curvature of the penis. Although many men have a slight natural curve to their penis, with true Peyronie’s disease the curvature is predominant when erect and is accompanied by pain. Following is an overview of the condition, the risk factors and possible treatments.

What is Peyronie’s Disease?

If you’ve been reviewing penis enlargement information, chances are you’ve seen the mention of Peyronie’s disease. Although mentioned on penis enlargement equipment manufacturer sites and in forums, oftentimes the term is just an ambiguous “penis curve.” What is Peyronie’s disease really?

According to the National Institute of Health, Peyronie’s disease is characterized by the formation of plaque within the penis. This plaque is actually a flat plate of scar tissue and typically develops on the top or bottom of the penis, inside the tunic albuginea. The tunica albuginea envelopes the erectile tissues, and although the plaque begins as localized inflammation, fibrous scar tissue begins to form and harden.  This may happen gradually over time, or it may occur literally overnight.

Symptoms of Peyronie’s Disease

As mentioned, many men have a natural, slight curve to their penis that is absolutely nothing to worry about. However, for the approximately 3 to 9 percent of men with Peyronie’s disease, following are symptoms that may be experienced, as noted by the Mayo Clinic:

  • Presence of scar tissue - The plaque or scar tissue, which occurs with Peyronie’s disease, can typically felt underneath the skin, as a band of hard tissue or a series of flat lumps.
  • A significant curvature of the penis - This curvature may be upward, downward, to the left or the right. This curvature usually begins where the hardening that can be felt is found. In some instance, when your penis is erect, it may have an ‘hourglass’ shape, with a narrow, tight band around the shaft.
  • Erection difficulties – For many men with Peyronie’s disease, erectile dysfunction occurs, with challenges getting or maintaining an erection.
  • Penis shortening – The penis may actually become shorter due to Peyronie’s disease.
  • Pain – Pain varies with Peyronie’s disease. Some men find that they only have pain when they are erect or only when they orgasm. Others experience pain anytime the penis is touched.

Risk Factors of Peyronie’s Disease

The condition is fairly uncommon and typically affects men 40 years old and older. Although the exact cause of Peyronie’s disease is unknown, there are a couple of risk factors.

  1. Penis fracture – Injury during intercourse can sometimes lead to Peyronie’s disease.
  2. Prostate cancer surgery or radiation treatment – The NIH has found that men who have undergone treatment for prostate cancer have a higher incidence of developing Peyronie’s disease.
  3. Duputren’s contracture – This is a cord-like thickening found across the palm of either one or both hands in men. Fairly common in white males over the age of 50, a small percentage of these men also develop Peyronie’s disease.
  4. Genetics – A certain type of immune cell marker has been found in men with Peyronie’s disease that may indicate there is a genetic predisposition to the disease.

 Treatment Options for Peyronie’s Disease

There have been a variety of treatments available for Peyronie’s disease.

  • Corticosteroid injections may be made into the fibrous scar tissue,
  • Potaba is an oral prescription medication sometimes prescribed,
  • Verapamil injections may be made into the fibrous scar tissue,
  • Collagenase is an enzyme that is currently under study for the treatment of Peyronie’s disease,
  • Xiaflex, a drug already approved for use for Duputren’s contracture, is in clinical trials, and
  • Radiation therapy and shock wave lithotripsy are less invasive treatment options.

Regretfully, many times these treatment methods are unsuccessful. As a result, for men who are unable to have sex due to their Peyronie’s disease, surgery is often considered. Risk factors with surgery include impotence, however, and therefore a penile implant is sometimes then warranted.

Traction-Based Extenders as a Treatment for Peyronie’s Disease

Traction-based extenders have been used by many men to help correct penis curvature and treat Peyronie’s disease. Through gradual, tension an extender gently straightens the penis. This traction can help break up the plaque that has formed as a result of the disease, and eventually dissolve the fibrous scarring mass. Although this is reported to not be a quick solution, when other non-surgical treatments have failed, it may be worth a try before going under the knife.

 


FDA Approves the 1st Drug Treatment for Peyronie’s Disease

xiaflex peyronies diseaseIn December 2013, the FDA approved the first drug treatment for Peyronie’s Disease — Xiaflex (collagenase clostridium histolyticum).

This is the first non-surgical treatment option the FDA has officially approved.

Peyronie’s Disease

Peyronie’s Disease is a curvature of the penis. Scar tissue under the penis’s skin causes the penis to bend abnormally during an erection. Although many men have a natural slight curve to their penis, Peyronie’s Disease is diagnosed when this curvature is at least 30 degrees during erection. This curvature can lead to problems during intercourse and even erectile dysfunction.

Xiaflex

Xiaflex is made from the protein of a living organism called – collagenease clostridial histolyticum. Xiaflex was first approved as a treatment for Dupuytren’s contracture — a progressive disease that affects a person’s ability to properly straighten and use the fingers of their hand, due to a build up of collagen. It is surmised Xiaflex also breaks down the build up of collagen scar tissue, that forms the curvature of the penis, for those suffering from Peyronie’s disease.

Treatment with Xiaflex

Treatment for Peyronie’s disease with Xiaflex is as follows:

  • A maximum of four treatment cycles.
  • Each treatment cycle consists of two injections of Xiaflex into the collagen-containing structure of the penis.
  • A penile modeling procedure (the massaging of the penis to help break up the collagen) is conducted 1 to 3 days after the second injection.

Xiaflex Studies

Two randomized, double-blind, placebo-controlled studies have demonstrated Xiaflex’s effectiveness for treating Peyronie’s disease.

  • 832 men, with Peyronie’s disease and a curvature of at least 30 degrees, participated in the studies.
  • Participants were given up to 4 treatment cycles of Xiaflex or the placebo and followed for 52 weeks.
  • Xiaflex was found to significantly reduce the penile curvature as well as the effects of Peyronie’s disease, when compared with the placebo.

Risks Associated with Xiaflex

Xiaflex for Peyronie’s disease is only available through a restricted program under a Risk Evaluation and Mitigation Strategy, through your doctor, due to the potentially serious adverse reactions. This can include penile fracture, where one of the penile bodies ruptures, and other serious penis injury. Other adverse side effects include: penile swelling, penile pain, and penile hematoma.

For this reason, Xiaflex should only be administered by a medical professional who has experience treating male urological diseases and who are certified within the program, by completing training in how to administer Xiaflex for Peyronie’s disease.

How to Use a Penis Extender

The problem with regarding “How to Use a Penis Extender” instruction guides up to this point is that many such guides do not give thorough information on what an extender user should do and experience to know they are on the right track towards optimal gains. This guide article covers the major points on how to do so. While there are some basic principles of “acquiring and mastering useful techniques” with your extender, it took me years to figure it out on my own. This article clarifies much of the guesswork of how to use penis extenders.

Applying the Penis Extender: “Strapping-In”

One of the first things to consider when you are about to begin using your penis extender, is whether or not you have a pronounced frenulum. Whether you are still intact, or if you are circumcised, maybe you have a substantial frenulum.

The frenulum can be very sensitive on some men, in which case a piece of cloth or gauze is worn to protect the frenulum from the hard surface of the extender’s cradle.

Gauze that sticks to itself is usually preferable because it stays in place once applied. A strip about 6-8 inches works well to make 2 complete revolutions around the penis just below the glans.

Now, this 4-step process is how the penis extender is applied to the penis.

  1. Make sure one side of the strap is fastened to the cradle of the extender, so that the strap will stay in place when applying the extender onto the penis. Usually the preferred method is to leave the strap unattached on the side of the extender that corresponds to your dominant hand, so if right handed, leave the right side of the strap unattached, and vice versa.
  2. Then, grab the penis just below the glans and with your opposite (non-dominant) hand and pull the glans towards the cradle of the extender. Once in place and with a grip still holding just under your glans, slide the strap up and over the penis behind the glans where it feels comfortable. Some men prefer some extra “slack” skin to be above the grip point of the strap for added comfort and to better protect the glans and frenulum.
  3. Now put the strap through the opening for it on its side of the cradle and secure it tightly while still keeping the penis held in place still by the other hand. Next, secure the other side of the strap to the extender with the dominant hand, making sure to keep enough tension on the penis so it stays in place and the glans does not slip out of the grip from the strap.
  4. Finally re-tighten the strap 2-3 more times on each side (one side at a time) to ensure a tight fit so that the glans in no capacity can slip out from the grip of the strap. Also be careful to not pull the strap so tight that it pulls shaft skin though the opening for the strap. This will be very painful, and you should feel it at once if this begins to happen.

Wearing the Penis Extender: The Waiting Game

Once everything is fastened and secured and you are well “strapped in” to the extender, as many say, it’s mostly an art of maintaining a comfortable stretch and doing things you enjoy to do while sitting or lying down. Much of the writing this article was done while wearing my extender. Many just enjoy watching TV, using their computer/laptop, or doing just about anything that does not require physical movement.

Some attest that lying almost flat on your back is the best way to keep the glans swollen with blood hence providing the best form of resistance for stretching against. This has come to be known as a “Head-Swell”, and by most is considered to be the one key factor in optimizing gains with extender use. How to get and maintain a “Head Swell” is fully covered in the following section of this guide.

The key here is to not move around so much that you loosen the grip of the harnessing strap, thus weakening the tension applied to stretching the penis. You obviously never want to over stretch, and sheer pain is the obvious best indicator of too much tension. You should be trying to get the maximum amount of tension you can without pain from the extender when wearing it. This is what causes gains to become maximized.

Tension + Time = Permanent Penis Shaft Length Gains.

Maximizing painless tension while wearing the extender is what helps optimize your gains.

This is done by first getting the extender on well so that you get a “Head Swell” and then by using the screw tension rods that connect the base of the extender to the glans cradle. Once the extender is secured well and the Head Swell is achieved, you should begin to slowly screw out the rods applying more tension to the shaft of the penis.

Personal research and interviews yields that the best way to do this is to twist the rods 3 revolutions every 10-15 minutes. You do not want to tighten it all the way out to quickly, because the penis shaft needs time to gain more plasticity and trying to do it too fast will cause pain and a loss of head-swell. Applying heat is always a good idea and very helpful to the process of penis extending if possible.

Applying Heat while Wearing the Penis Extender

You can apply a warm rice sock to the penis shaft while extending in order to not only help the penis achieve a more plastic/malleable state more quickly, it also helps reduce the time for extending the extender rods. This allows you to achieve maximum tension, or your BPXL (Bone Pressed eXtender Length) more quickly.

Applying heat also helps when you begin using an extender because it can help prevent injury. The same heat sources recommended for manually exercising the penis can be used while using an extender as well. However, liquid heat should never be used, as it could contribute to oxidation (rusting) of the metal parts of the extender. It can also help add to chaffing if you are using gauze or cloth as a barrier between your penis and the extender.

Optimal Maximized Tension

Once you have screwed out the side rods so that no more tension can be applied to your penis without any pain, stop. You are now at the point where you have max tension and gains are optimized. Trying to overdo it from this point will only result in injury, hence detracting from the purpose of wearing the extender.

In the beginning, never over extend the tension rods. Doing so can cause pain and lead to injury. It can also destroy your extender if you are not careful. As extenders are not cheap products like some other penis enlargement devices, it is important to be careful with your extender and not explore the limits of its resilience. They are fragile and thus is why public use is not recommended.

Take care of your extender and it will continue to give you the gains you desire. Although it takes time, penis extenders are proven to work and will definitely deliver permanent enlargement of the penis!

The Head Swell: Maximize Your Penis Extender Gains

This section covers a particular phenomenon that can occur while wearing a penis extender called a “head-swell”. It is now believed by many that the head-swell helps to optimize penis shaft length gains while wearing a penis extender. However, some disagree or say that a head-well is not necessary for making gains when using a penis extender. Nonetheless, this article focuses on how to get and maintain a head-well while wearing a penis extender.

What is a Head-Swell?

Best defined, a “Head-Swell” is a blood-engorged glans that occurs while wearing a penis extender which allows the penis extender to be worn at appropriate tension levels. Appropriate tension is best defined as enough tension so that there is level of tightness in the middle of the penile shaft when the extender is properly worn with a head-swell.

A head-swell is also believed to help the penis from going cold or numb while wearing a penis extender, since it means that the glans will be full of blood. This is not proven, but I have noticed that indeed, a head-swell does seem to allow me to wear my extender for a longer amount of time without cold or numbness occurring.

I am a firm believer that using a penis extender is different for every man, and that it is not an exact science (yet). Each man must figure out what works best for him, but the ability to get a head-swell is a very good skill to possess if one is planning to use a penis extender.

How to Get a Head-Swell: Proper Application is Key

Let us review quickly how the extender is applied, which will segue into obtaining a head-swell. Remember, proper application is the first step to achieving a head-swell! You will begin to notice that the better you become at putting the extender on with a good deal of blood already in your glans, the easier and better your head-swells will be.

First, get about a 30% erection, and have your extender ready to apply. Now, do some helicopter shakes and make sure your penis is not at all “hard” per se, but is not turtled in any way either, and is moderately engorged with blood. Good, long and loose is what you want.

As a reference point, my penis will only stay like this for literally a matter of seconds before it loses all “flushed flaccid” characteristics which is why having you extender and any wrapping (gauze or cloth) ready to apply.

Head-Swell Focus

Put the extender on by grabbing mid shaft to a quarter length of the shaft from the glans and pull some loose skin of towards your glans in a somewhat jelqing fashion, keeping as much blood in your glans possible. At this point secure your fastening strap to the shaft.  Then secure the penis to the extender and cinch down this right side.

Now, having the right side somewhat pretty tightly fastened, unfasten the left side and re-fasten and re-tighten the left side of the extender. At this point, the left side is only barely secured to the extender to give you something to pull against when setting the initial right side.

Next, make a final adjustment to each side and when you can no longer pull the strap, you are “Strapped-In”. You are now ready to focus on the final steps of achieving a head-swell.

Kegeling and Jelqing to Get Head-Swells

Immediately after you get it on, one method that is most common is to use the kegel to force even more blood into the glans. Do this by simply kegeling once you have the extender on, but also use gravity to your advantage by making sure that your penis and the penis extender are pointing downward while you begin kegeling.

This generally works well, but I have found that there is a portion of times that kegeling will not effectively produce a head swell. This is when I thought, “Why not try to manually use a jelqing motion to get more blood in the glans to better create a head-swell?”

So, I have found that even if I do not get the extender on with adequate blood in my glans at all, and if kegeling fails to force blood into my glans, by using my thumb and forefinger to do a quasi-jelq up the shaft with the extender on I can get a head-swell. You should also be very careful as to not touch the side bars while jelqing a head-swell or you could break your extender by squeezing the rods. Extenders are expensive! So take special care to only touch your penis shaft when attempting the “Jelqing a Head-Swell” maneuver.

Keeping a Head-Swell and Proper Tension

As it may be best for me to fasten it standing up, maintaining a head-swell is hard while standing for long periods of time. Some believe that lying down is best for keeping a head-swell. I have no problems maintaining a head well while sitting up, but standing for too long or walking around can surely cause a good head swell to diminish. Try not to move around too much once you have the extender securely on your penis with a good head-swell.

I do well also by checking frequently to see if I am losing the head-swell. If I am, I simply twist/screw-in my torsion screw bars so that they are back in their original (shortest) position, then without taking the extender off, I simply re-jelq my head-swell, and it generally works well. If not, you will just have to take off the penis extender and wait a while before trying to reapply it.
Also remember that it takes practice.

Proper use of a penis extender can be much like learning to play the guitar, or use a tool well. It takes some time to get accustomed to wearing your penis extender flawlessly every time. And even then, just when you think you have it figured out, we all have a problematic session every now and again. Just don’t get discouraged and stick with it!

This section of the penis extender guide explains the necessity of creating a penis extender routine that will fit into your schedule. This section also will cover how you should track your extender sessions and gains with a log, as well as the best ways to incorporate using an extender with your manual penis exercises.

Now What?

Now that I have covered how to properly use a penis extender, it is necessary to develop a routine based on your daily schedule, goals for gains, and proper implementation. Since this site is devoted mainly to the manual stretching and jelqing techniques and exercises, I find it safe to assume a vast majority of the readers here are mainly looking for a way to add extending to your PE routine already in place, so this will also be covered. First, there are some things to consider.

Finding Time

If you will recall from the first section, extending can be quite arduous and in my humble opinion and for safety reasons, using a penis extender should really not be done while driving or in any public situation because of possible injury as well as cheapening your gains and wasting your time with not enough tension. Let me put it another way: If you can walk around normally with your extender on, then you are most likely doing it incorrectly or in a fashion that will not produce maximum gains for the time spent extending.

This means that you will have to find some quasi-private time when you can wear only a towel, very loose sweat pants or preferably nothing at all while you use your penis extender. Now, the fact that you have the time to be here reading this article/website generally assumes that you have at least a few private hours to yourself by which you could possibly also be wearing an extender and making blog posts, as in, right now!

You Really Do Have the Time

Face it, you have the time to devote to extending if you can sit in front of a computer and read this article now in private, which is likely the case. Now, if you have already spent the money on an extender (they are not cheap) and you are reading this article, it shows that you must want to put in some time and get the most out of your penis extending sessions.

One great thing about extending is that you can occupy yourself with other things while you wear the penis extender, so multitasking is definitely a possibility. But don’t plan on getting in your daily run or weight room session while you use your penis extender, you just can’t do so. However, any activity where you will be sitting or lying down for 98% of the time is perfect time for using your penis extender.

Extender Routine 101

So we have already covered the main issues, those being that you will need at least enough privacy to open your pants every 10-15 minutes to check for numbness and increase/adjust the tension.
Now, you will want to consider what times are best and when you will be performing actions like sitting at the computer, watching TV or doing nothing at all, that is, times each day when you can count on being able to use/wear your extender.

The key element to keep in mind regarding these time periods for extending is consistency. There may be instances for “extra time” that you can get an extra hour here and there, but the key thing to remember with your baseline routine are the times that you know you will be able to use it.
And it’s ultimately up to you to decide if you want a bigger, thicker penis. With proper technique and patience, you will begin to notice your BPXL begin to increase, and that you can hold a head-swell at longer lengths for longer periods of time.

Combining Penis Extending with Manual Penis Enlargement Exercise Sessions

One thing many guys ask on the PEGym forums is “How can I use my penis extender in combination with my manual penis enlargement exercise sessions that I currently do?” Well, this is not an easy task, especially once you consider first all the time it takes out of your day to do the other necessary things like work, school, and housework. However, there are some even more important things to consider.

One thing that is important to consider is when best to use the extender in relation to manual exercises. I have found that it has made an extreme difference in flaccid hang and I imagine eventual gains by wearing my extender immediately after my complete stretching/jelqing sessions. This is because of how it keeps the penis in an extended state after a rigorous workout.

I also think that it is not bad to use the extender before, but to avoid injury I would not wear the extender an hour or less before a manual penis exercises, as to not put too much stress on the penis while exercising with adequate intensity.

The Power-Hour

One concept that bgstick18 and I have developed is the idea of the “Power Hour”. This concept is that of wearing a penis extender for an hour at full tension then taking it off. This is only advised for men that have used a penis extender for 6 months to a year and are well accustomed to extreme stretches. We believe that your BPXL should be at least an inch longer than your BPEL before attempting a power hour.

When doing a power-hour, the idea is to get your penis to its maximum BPXL immediately after putting on the extender and getting a head-swell. Sure, it may be possible to add even more tension throughout the power-hour, but the idea is that you get pretty close to max tension right from the beginning.

I have found that by doing this right after manual exercises is making a huge difference in the way my penis feels, and I hypothesize that maximum optimal gains will ensue! More research needs to be done on the power-hour and how it helps/affects overall gains.

Final Note

Your routine for wearing your penis extender is a critical element in achieving gains and should be a well thought out plan. By organizing the free time in your day you can really optimize the time spent wearing your penis extender. And by then incorporating your penis extender routine with a manual exercise regimen, it is believed that even more gaining potential will manifest.

Bone-Pressed eXtender Length: Understanding Your BPXL

Wearing a penis extender can be a complicated process. More over, understanding gains made with a penis extender can also be baffling because the difference in your overall BPEL is usually less than the length that can be reached while wearing a penis extender. The solution? Track your gains consistently and accurately by measuring your Bone-Pressed eXtender Length, or BPXL.

Bone-Pressed eXtender Length Explained

A man’s “BPXL” is a new term coined by PEGym community member Dangler. You might have seen the term on our penis forums involving penis extenders. The BPXL is the term we now use to describe Bone-Pressed eXtender Length. There are a few elements that come into play here other than just a simple measurement taken while wearing the extender.

The first thing that must be lucid is that you measure bone-pressed just as you normally would with any other measurement. Hence, you do not measure from the base of the penis extender; you measure from how far you can press the ruler into the pelvic bone just outside of the penis extender’s base.

The next important factor is to know when to take the measurement. The measurement is taken when you have been wearing the extender for an hour or more without increasing the tension, meaning that you can “hold” that BPXL for at least an hour without numbness. Coldness and discomfort are not factors in measuring for BPXL, and can be tolerated but not in excess.

To measure BPXL, push the ruler or measuring device into your pelvic bone on the outside of the extender base just as you would to get your BPEL.

Measuring BPXL and Logging Hours

From now on, you should measure gains made with a penis extender while wearing the extender. However, remember that this is now your BPXL (Bone-Pressed eXtended Length) and should not be confused with Bone-Pressed Erect Length, your Bone-Pressed Flaccid Length, or other forms of penis measuring.

The reason for these separate measurements is because it can help you see exactly the length to which you can extend as well as for how long you held that stretch as you track your penis enhancement gains.

Again, your BPXL is measured by two factors:

  • First is your bone-pressed length while wearing the extender.
  • Second, it is the maximum extended length that you can maintain for at least a whole hour without going numb or causing any real pain.

Remember, if you extend to a length that you can’t even hold for 5-10 minutes, then you are just cheating yourself, and you will eventually become discouraged. Not only is this lying to yourself, but cheating in this manner also will not produce or reflect real quality penis growth, which takes time.

It is best to have a clock beside you when measuring your BPXL, or use the clock on your computer if you make a log using software. I use an MS Excel spreadsheet with which you can implement a formula that will automatically add up the time that I spent using my penis extender.

It is also a good idea to make notes regarding any thing that happened during each session, like if you experienced any numbness, if you were able to increase your BPXL, or if you were able to wear your extender for a longer period of time at your current BPXL.

The Key: Consistent BPXL Measurement Procedures

One of the most important things to keep in mind when taking a BPXL measurement is how you performed your last measurement. Exactly how long did you wear the penis extender? Did you feel any pain when you last measured?

You should really use your best intuition to try and mimic the exact conditions of your previous measurement when performing a new, accurate BPXL measurement.

It is very important to keep in mind that you are not trying to impress anyone. You are enlarging the penis, and attempting to make penis shaft length and girth gains. To observe and track these penis enlargement gains is why using the BPXL measurement is a very useful measurement.

Your ability to add more and more tension to your penis while wearing the extender will always mean that your BPEL has increased. However, the length of your BPXL gains do not directly translate into the same length gains for your BPEL, hence the need to have the new BPXL measurement system for measuring progress with a penis extender over time, and keeping the BPEL its own separate measurement.

Tunica Hanging and More on Penis Hanging Angles

This penis hanging guide section covers more in-depth discussions regarding the use of various angles when hanging. This is especially pertinent information when shifting the focus of your hanging from the suspensory ligaments to the tunica, which is considered the main limiting factor with penis enlargement gains.

SUSPENSORY LIGAMENT MAINTENANCE

If you did not naturally start with your penis exiting your pelvis at 6’ Oclock, you will need to do some maintenance work until you ‘cement your gains’. This means, you will need to practice some of your penis hanging at least a few sets per week at a lower angle: straight down (SD) or between the cheeks (BTC) to maintain your ligament stretch gains.

If your suspensory ligaments tighten, pulling your package upward to a higher angle (e.g. 6.5 O’clock or 7 O’clock), you will need to go back to hanging BTC or SD again (at least part time) until this is corrected.

Eventually your penis will stay at this downward angle and your suspensory ligament stretch gains will be ‘cemented’. No one can really tell you how long this maintenance will take since it depends on your physiology and how long it took you to reach 6’Oclock as well.

Once you’ve maximized your suspensory ligament stretch gains, you’re ready to start targeting the penis tunica.

TUNICA HANGING WORK

The tunica is probably the toughest single structure in our penis. Through your hanging career, the tunica will slowly toughen and become thicker. As you stretch the tunica and permanently elongate it, elasticity will slowly be lost and making permanent tunica hanging gains will slowly become more difficult. Bib (aka Bigger), an expert on hanging recommends the RSDT fulcrum for tunica hanging deformation process.

TUNICA HANGING: Straight Out, Straight Up, Over the Shoulder

If you’ve just started hanging and your penis exits your pelvis at 6 O’clock (straight down), you might not benefit from SD or BTC angles as much as others. However, you should still focus on tunica hanging via  BTC for at least a month to make sure your suspensory ligaments are fully stretched.

Once you have confirmed you have no more potential ligament stretch gains, you can move to OTS (over the shoulder) or SU (straight up). These are probably the two best angles for someone starting out with their penis naturally exiting the body at 6 O’clock. To hang OTS use a plastic milk jug to protect your shoulder (otherwise the rope or nylon strap from your penis hanger will eventually dig in and start to hurt).

If you’re using an OTS angle for tunica hanging, alternate shoulders with each set. If you’re doing an odd number of OTS sets, finish off with an hanging angle in the middle such as straight up (SU). Once you’ve had at least six months of penis hanging experience, and your penis exits your body at 6 O’clock (straight down) you will be ready for advanced Tunica work, including fulcrums such as the RSDT.

THE “RICE SOCK DUCT TAPE” FULCRUM FOR TUNICA HANGING

The RSDT (Rice Sock Duct Tape) Fulcrum is an excellent way to create tunica creep, micro-tears and eventually permanent deformation. By the time you’ve maximized your suspensory ligament stretch gains, you may have one year or more of hanging experience under your belt. The tissues of your tunica will be toughened and no longer easy to deform. The RSDT is an excellent fulcrum for breaking through plateaus and creating fatigue all along the entire penile shaft.

Effectively the RSDT is a rounded surface (like a small mound) with channels on the sides that help your penis shaft lay straight. Bib, an expert on hanging wrote an article, complete with pictures on how to create an RSDT here: How to Build the RSDT

The RSDT is a very intense fulcrum that will create fatigue all along your shaft. At the start, you will only be able to hang a fraction of your normal weight using this device. However, you’ll reach excellent levels of fatigue quickly, and reports suggest penis length gains with this exercise are exceptional.

TUNICA HANGING FULCRUMS AND PEYRONIE’S DISEASE

If you have Peyronie’s disease, a fulcrum is an excellent way to permanently break up the scar tissue and curving, restoring your natural penis. Dr. Howard did this successfully with hanging after he found a penis extender was not capable of correcting his condition. ANGLE OF ATTACK FOR TUNICA HANGING AND FATIGUE

With a myriad of tunica hanging angles to choose from and fulcrums like the RSDT, you can break through any plateau and reach your tunica hanging length gains. Knowing which angle of attack (or fulcrum) to use is critical in achieving your penis length goals with tunica hanging. However, penis enlargement through hanging routine is sometimes more art than science.

Having a good routine and using the right angles is not quite enough, you must also fatigue your penile tissues to make gains. FDA Approves the 1st Drug Treatment for Peyronie’s Disease

xiaflex peyronies diseaseIn December 2013, the FDA approved the first drug treatment for Peyronie’s Disease — Xiaflex (collagenase clostridium histolyticum).

This is the first non-surgical treatment option the FDA has officially approved.

Peyronie’s Disease

Peyronie’s Disease is a curvature of the penis. Scar tissue under the penis’s skin causes the penis to bend abnormally during an erection. Although many men have a natural slight curve to their penis, Peyronie’s Disease is diagnosed when this curvature is at least 30 degrees during erection. This curvature can lead to problems during intercourse and even erectile dysfunction.

Xiaflex

Xiaflex is made from the protein of a living organism called – collagenease clostridial histolyticum. Xiaflex was first approved as a treatment for Dupuytren’s contracture — a progressive disease that affects a person’s ability to properly straighten and use the fingers of their hand, due to a build up of collagen. It is surmised Xiaflex also breaks down the build up of collagen scar tissue, that forms the curvature of the penis, for those suffering from Peyronie’s disease.

Treatment with Xiaflex

Treatment for Peyronie’s disease with Xiaflex is as follows:

  • A maximum of four treatment cycles.
  • Each treatment cycle consists of two injections of Xiaflex into the collagen-containing structure of the penis.
  • A penile modeling procedure (the massaging of the penis to help break up the collagen) is conducted 1 to 3 days after the second injection.

Xiaflex Studies

Two randomized, double-blind, placebo-controlled studies have demonstrated Xiaflex’s effectiveness for treating Peyronie’s disease.

  • 832 men, with Peyronie’s disease and a curvature of at least 30 degrees, participated in the studies.
  • Participants were given up to 4 treatment cycles of Xiaflex or the placebo and followed for 52 weeks.
  • Xiaflex was found to significantly reduce the penile curvature as well as the effects of Peyronie’s disease, when compared with the placebo.

Risks Associated with Xiaflex

Xiaflex for Peyronie’s disease is only available through a restricted program under a Risk Evaluation and Mitigation Strategy, through your doctor, due to the potentially serious adverse reactions. This can include penile fracture, where one of the penile bodies ruptures, and other serious penis injury. Other adverse side effects include: penile swelling, penile pain, and penile hematoma.

For this reason, Xiaflex should only be administered by a medical professional who has experience treating male urological diseases and who are certified within the program, by completing training in how to administer Xiaflex for Peyronie’s disease.
